Skilled Nursing
Care Origin offers access to licensed registered nurses to monitor patient’s
healthcare needs, educating each patient and caregiver while collaborating with
the attending physician.

Physical Therapy
Our physical therapists help patients regain functional mobility through
therapeutic exercise, balance activities and ambulation training.

Occupational Therapy
Our occupational therapists use fine motor, cognitive, perceptual and sensory
exercises and tasks to help patients regain daily living skills.

Speech Therapy
Our speech language pathologists help patients regain their ability to produce
and understand speech as well as facilitate communication skills and manage
swallowing disorders.

Medical Social Workers
Medical social workers evaluate the social and emotional the social and
emotional factors that affect patients and their families, by managing stress
and other challenges brought on by the illness or disability, using the many
community resources available on them.

Home Health Aides
Care Origin offers Home Health Aide (HHA) services to assists patients in
optimizing their activities of daily living (ADL). Our HHA’s are specially
trained to deliver many complex services under the direction of a registered
nurse.

Registered Dietician
Our registered dieticians provide nutritional information and support to our
patients.

To Arrange for Services
A patient must be under the regular care of a physician, home bound and in need
of intermittent skilled nurse care, physical or speech therapy or have an on
going need for occupational therapy.
Our services may be requested by a patient, physician, friend,
family, Insurance company or discharge planner.
Care Origin also assists the patients in procuring durable
medical equipment, such as hospital beds, wheel chairs, walkers, oxygen,
infusion pumps or any other supplies needed by the patient.
